2015-16 PARENT/ATHLETE MEETING

MONDAY, AUGUST 3 @ 6:30 P.M.

DODGEVILLE HIGH SCHOOL GYM

ALL FALL ATHLETES MUST ATTEND THIS MEETING WITH A PARENT TO BE CONSIDERED ELIGIBLE TO PARTICIPATE.  

PLEASE BE SURE TO SIGN IN AT THE APPROPRIATE TABLE SO YOUR ATTENDANCE IS ACCOUNTED FOR.  

*WINTER AND/OR SPRING ATHLETES MAY ATTEND THIS MEETING AS WELL.  ATHLETES ONLY NEED TO ATTEND ONE MEETING PER YEAR BEFORE STARTING THEIR CHOSEN SPORT(S).  

 

 

 

CODE OF CONDUCT FOR ATHLETIC AND CO-CURRICULAR ACTIVITIES

Dodgeville High School offers the following athletic activities:

FORMS NEEDED TO PARTICIPATE

All athletes need to have the following forms on file in order to practice & compete in any DHS sport:

 

-WIAA PHYSICAL FORM
The WIAA requires that all student athletes have a physical every 2 years.
-WIAA ALTERNATE YEAR FORM
The WIAA requires athletes to have a parent sign this form every year opposite the physical.
-DHS ATHLETIC REGISTRATION FORM
In addition to either the physical or alternate card, this is the only form that needs to be completed, signed and returned to the AD Office. Please be sure to read the WIAA Eligibility Bulletin and the Concussion Fact Sheets (listed below) before signing off on page 2 of this form.

This form does not need to be returned. Please read and sign off on the DHS Athletic Registration Form to certify that you have read, understand and agree to abide by all of the information contained in this bulletin.
---WIAA Concussion Fact Sheet for Parents
Please read and sign off on the DHS Athletic Registration Form to certify that you have read the Concussion and Head Injury Information and understand what a concussion is and how it may be caused.
